Click below to download the application.  Once complete, return to the HRA at 222 E 2nd St, Duluth, MN 55805 along with your $40 application fee. Please note, Fairmount Cottages accepts check or money order only for application fees, and cash will not be accepted. 

If you do not have printing capabilities you may submit online via email, however your application will not be accepted until the $40 fee is received.
